Civil Service

Upon initial appointment, the Title-42 fellowship entry grade will be set comparable to the Federal Government General Schedule (GS) base pay rate equivalent of GS-12, step 3 regardless of previous experience and compensation. An additional locality pay is based on the location of the official duty station. Physicians may also receive an additional $20,000 per year with an active, U.S. unrestricted license.

USPHS Commissioned Corps Information

The USPHS is one of the seven Uniformed Services. It is a specialized career system designed to attract, develop, and retain health professionals to accomplish its mission.

Incoming EIS officers must meet eligibility criteria for the Commissioned Corps:

  • Be a U.S. citizen
  • Be less than 44 years of age (at date of entry into EIS)
  • Have a degree in an appropriate health-related discipline
  • Meet medical standards

Student Loan Repayment

A limited number of EIS officers may be eligible to receive student loan repayment, based on availability of funding, for a 2-year service agreement. Only federally made, insured, or guaranteed student loans are eligible for repayment consideration. Consolidated loans of both an EIS officer and spouse would not be eligible.

Active Duty Pay Uniformed Service Members - The Basics

Basic Pay, Basic Allowance for Housing (BAH), and Basic Allowance for Subsistence (BAS) are the fundamental components of military pay. Officers who are married or have legitimate dependents are paid at a higher rate.

The Department of Defense Military Compensation website addresses military pay and benefits for current members, retirees, and survivors of retirees. Provided by the Office of the Under Secretary of Defense for Personnel and Readiness the site defines the compensation available to uniformed members.

Special and Incentive Pay
Officers are also compensated financially for circumstances such as separation from family, hazardous duty, and assignment to specific duty locations. In addition to basic pay elements, a special, incentive, or critical skills pay is given to qualified officers who possess specific or unique skills, or ones at a critical shortage. PHS utilizes Health Professions Special Pay (HPSP) to compensate officers possessing unique skills to retain them for specified periods of time. Clothing Allowance
Officers new to Active Duty are eligible to receive a one-time uniform allowance to offset the initial expense of purchasing a new wardrobe of uniforms.

Basic Allowance for Housing (BAH)
BAH is an allowance to offset the cost of housing when officers do not receive government-provided housing. BAH depends upon duty location, pay grade and whether the officer has dependents. BAH rates are set by surveying the cost of rental properties in each geographic location. Therefore, BAH rates in high-cost areas will be much greater than those in low-cost areas. Overseas Housing Allowance (OHA)
The Overseas Housing Allowance, or OHA, is paid to officers who live in private housing at overseas duty station. OHA helps offset housing costs, which are made up of rent, utility and recurring maintenance expenses, and move-in housing allowance (MIHA). Basic Allowance for Subsistence (BAS)
BAS is meant to offset costs for a member's meals. This allowance is based in the historic origins of the military in which the military provided room and board (or rations) as part of a member's pay. This allowance is not intended to offset the costs of meals for family members.

Servicemembers Group Life Insurance (SGLI)
Officers automatically have the maximum $400,000 of SGLI coverage. Officers can decline or elect lesser amounts of coverage in writing in increments of $50,000. SGLI coverage stays in effect for 120 days after discharge. Members who have SGLI automatically have Traumatic SGLI (TSGLI).

Family Servicemembers' Group Life Insurance (FSGLI)
Provides group term life insurance coverage to the spouses and dependent children of Officers insured under SGLI. The Officer pays the premium for spousal coverage. Dependent children are insured at no cost.

Thrift Savings Plan (TSP)
The Thrift Savings Plan (TSP) is a Federal Government-sponsored retirement savings and investment plan. The National Defense Authorization Act for Fiscal Year 2001 extended participation in the TSP, which was originally only for Federal civilian employees, to members of the uniformed services, and members began enrolling on October 9, 2001. The TSP offers the same type of savings and tax benefits that many private corporations offer their employees under so-called "401(k)" plans. The retirement income that you receive from your TSP account will depend on how much you have contributed to your account during your working years and the earnings on those contributions. Blended Retirement System (BRS)
Officers called to active duty after Dec. 31, 2017 - are enrolled in the BRS. The BRS includes automatic government contributions of 1 percent of basic pay and government matching contributions of up to an additional 4 percent of basic pay to an officer’s TSP account. Officers nearing ten years of active duty service, can elect to receive Continuation pay. Continuation pay is a direct cash payout (like a bonus), in return for additional obligated active duty service. Changes to the Uniform Services’ retirement system went into effect January 1, 2018. Compensation

Your starting base pay as a Public Health Service officer is competitive with industry starting salaries and increases with promotions and years of service.

Total compensation

Total compensation varies depending on factors such as education and training, professional experience, and geographic location of assignment. While it is somewhat difficult to compare your take-home pay with someone in the private sector, your base pay, coupled with a generous assortment of nontaxable income and benefits, provides a very competitive compensation package that grows with years of service and promotions.

Health Professions Special Pays (HPSP)

If you are a healthcare professional serving as an officer in the Public Health Service, you enjoy a wide range of opportunities and financial incentives. HPSP is available if you choose to serve full-time on Active Duty or part time in the Ready Reserve. 

Active Duty Rates: 

Accession Bonus (AB), Critical Wartime Skills Accession Bonus (CWS-AB) (Signing Bonus)

  • Medical Category: (CWS-AB) When reporting to initial Active Duty, you qualify for this bonus if you are in a designated specialty. Bonuses range from a total payment of $200,000 to $400,000 depending on the specialty.
  • Dental Category:(CWS-AB) When reporting to initial Active Duty, you qualify for this bonus if you are in a designated specialty. Bonuses range from a total payment of $150,000 to $300,000 depending on the specialty.   
  • Veterinary: (AB) When reporting to initial Active Duty, you qualify for this bonus if you are in a designated specialty. The bonus is $20,000.    
  • Health Services Officer (Physician Assistant): (AB) When reporting to initial Active Duty, you qualify for this bonus if you are assigned to a clinical position in a designated specialty. The bonus is $60,000.
  • Health Services Officer (Clinical Psychologist): (AB) When reporting to initial Active Duty, you qualify for this bonus if you are assigned to a clinical position in a designated specialty. The bonus is $60,000.  
  • Scientist (Clinical Psychologist): (AB). When reporting to initial Active Duty, you qualify for this bonus if you are assigned to a clinical position in a designated specialty. The bonus is $60,000. 
  • Nurse: (AB) When reporting to initial Active Duty, you qualify for this bonus if you are assigned to a clinical position in a designated specialty. The bonus is $30,000.   
  • Pharmacist: (AB) When reporting to initial Active Duty, you qualify for this bonus if you are assigned to a clinical position in a designated specialty. The bonus is $30,000.   

Board Certification Incentive Pay (BCIP)

  • Is a $6,000 annual incentive paid to accredited specialties recognized by the USPHS Commissioned Corps.  

Retention Bonus (RB)

  • Is a bonus paid to designated specialties (Physicians, Dentists, Veterinarians, Nurse Practitioners, Nurse Midwives, Certified Registered Nurse Anesthetists (CRNAs), Optometrists, Physician Assistants and Psychologists).  Bonus rates vary based on the specialty and the number of additional years you agree to complete.  

Incentive Pay (IP)

  • Is an incentive pay for designated specialties (Physicians, Dentists, Veterinarians, Certified Registered Nurse Anesthetists (CRNAs), Optometrists and Pharmacists).  Bonus rates vary based on specialty.  

Assignment Pay (AP):

  • Is an incentive pay for designated specialties at select locations.  Rates vary based on the specialty, location and length of assignment.

Education

The USPHS Commissioned Corps understands the financial burden of an education. Certain Federal agencies may offer loan repayment and other educational and family support programs to you as a Public Health Service officer.

Education and loan repayment benefits

The Indian Health Service awards up to $40,000 in loan repayment to health professionals in exchange for at least two years of service in health facilities serving American Indian and Alaska Native communities. Learn more

The National Health Service Corps (NHSC) offers select medical professionals $60,000 in loan repayment in exchange for two years of service at a community-based site in a high-need health professional shortage area. Recipients must apply to and accept a position at an NHSC site. Learn more

The Centers for Disease Control and Prevention (CDC) Epidemic Intelligence Service (EIS) Program provides federal loan repayment in exchange for two years of service practicing epidemiology at the CDC or with state/local health departments. The Health Resources & Services Administration (HRSA) Nursing Education Loan Repayment Program is a selective program that helps alleviate the critical shortage of registered nurses currently experienced by certain types of healthcare facilities by helping nurses working at them to repay their student loans. If you are assigned to active duty in one of the states with no income tax, you can establish residency in that state for the rest of your uniformed service career.
  • Retirement Benefits: One of the key benefits of serving in the uniformed services is the retirement plan. If you accumulate 20 or more years of active duty service, you are eligible for retirement. If you are a Ready Reserve member, if you accumulate 20 or more years of qualifying service, you are eligible for reserve retirement pay in most cases at age 60.
  • Blended Retirement System (BRS):BRS includes employer paid contributions into your Thrift Savings Plan (TSP) account that you manage. In retirement you will receive a percentage of your basic pay for each year of service, BRS uses a multiplier that is 2.0% times the number of years of service times your highest 36 months of basic pay.
  • TSP: TSP is a retirement saving and investment plan similar to a 401(k) plan. You will receive a government contribution that equals 1% of basic pay to a tax-advantaged retirement account (TSP) after 60 days following entry to duty. Additionally, you have the ability to receive up to an additional 4% matching contribution from the government to TSP after the 2nd year of service through the 26th year of service. 
  • Relocation Expenses: You will receive paid relocation when you join the USPHS Commissioned Corps. You will also receive relocation expenses when relocating between duty assignments. Relocation expenses are also paid upon retirement or separation (based on your home of record).
  • Travel Expenses: You will receive paid expenses for travel related to your job.
  • Military Lodging and Morale, Welfare and Recreation (MWR): You will have access to military base lodging and MWR facilities.
  • Military Stores: You will receive shopping privileges at military base grocery and department type stores.
  • Military Space Available (Space A) flights: You and your family will have access to Space A flights domestically and internationally.
  • Banking and Insurance Products: You will have access to exclusive insurance and banking products targeting the needs of active duty uniformed service members.
  • VA Benefits: You will have access to a host of VA benefits such as the VA Loan Guaranty Home Loan Program, VA Disability benefits, Group Life Insurance, and VA Survival and Death benefits as applicable.
